Anonymous feedback form
Give people a place to say what feels difficult to say out loud. Create a form, share the link, and collect feedback without asking for a name or email address.
What is an anonymous feedback form?
An anonymous feedback form lets people share suggestions, concerns, or experiences without attaching their identity. Instead of putting someone on the spot in a meeting or asking for a named email, you give them a separate place to speak up.
In InnerVoice, that form is called a suggestion box. You choose its name and description, then share its link. Responses arrive in your dashboard; they do not automatically appear on a public wall.
How to create one
- 1
Create your suggestion box
Sign up, give your box a name, and add a short invitation explaining what you want to hear about.
- 2
Share its link
Send the link in a message or email. People open the form and submit without creating an account.
- 3
Bring it into your space
Download a QR code or printable poster for a staff room, reception desk, noticeboard, or classroom.
Feedback doesn't have to be the end of the conversation
"The new schedule is making things harder." That tells you there is a problem, but not enough to understand it. With private anonymous conversations, you can ask which part of the schedule is causing difficulty, and the sender can answer without revealing their name. Read how follow-up questions work.
What should you ask?
Start with a simple invitation rather than a long list of questions:
- What is making your day harder than it needs to be?
- Is there something we could do differently?
- What is working well that we should keep doing?
Avoid asking for identifying details. A thoughtful response matters as much as the form: acknowledge the concern, ask for context, and explain what you can do next. See our guide to handling hard feedback.
Common questions
Can I create an anonymous feedback form for free?
Yes. The Free plan includes one suggestion box with unlimited submissions, one anonymous poll, wall view with admin replies, and a daily email digest.
Will everyone be able to see the responses?
No. Feedback goes to the box owner's dashboard. You decide whether to enable a public wall and which notes or conversation messages to share there.
Do people need an account to submit feedback?
No. People open the form and submit without creating an account, and without giving a name or email address.
Does anonymous mean I should share any information?
No. Names are not required, but details someone writes or attaches may still identify them. Encourage people to leave out personal details they do not want to share.
